Position Overview:

We are seeking Family Nurse Practitioners (FNPs), Adult Nurse Practitioners (ANPs), and Adult-Gerontology Nurse Practitioners (AGNPs) to join the NYIA Independent Practitioner Panel in New York. This panel is responsible for completing the Community Health Assessments for consumers to become eligible for community-based long-term care services and supports.

As a practitioner, you will conduct follow-up in-home clinical visits for individuals who have undergone assessments. Comprehensive virtual training is provided and compensated.

 

Compensation:

  • Hourly Rate: $115.00 per hour

 

Requirements:

  • Board certification and personal medical malpractice insurance
  • Active NYS Medicaid ID number
  • Ability to conduct in-person visits in the community
  • Must be conflict-free, with no affiliations to:
    • Managed Long Term Care (MLTC)
    • Licensed Home Care Services Agencies (LHCSA)
    • Certified Home Health Agencies
    • Insurance companies or Institutional Special Needs Plans (ISNPs)
  • Minimum 2 years (3,600 hours) of relevant experience

 

Available Schedules:

  • Monday – Friday: 7:30 AM – 8 PM
  • Saturday & Sunday: 10 AM – 6:30 PM
  • Minimum commitment: 15 hours per week; available hours – up to 40 per week
  • Part-time schedules are available; that schedule is an in-person position only. No remote work is available for part-time hours

 

Training Requirements:

  • Week 1
    • 40 hours of instructor-led classroom training
    • Monday through Friday, 9:00 AM – 5:00 PM
  • Week 2
    • Approximately 35 – 40 hours of additional training, including:
    • Shadow visits
    • Quality assurance reviews
    • Conducting precepted assessments

Job Responsibilities:

  • Conduct initial in-home evaluations of patients using the UAS-NY assessment tool to assess health status, strengths, and care needs.
  • Develop and maintain positive relationships with consumers, caregivers, and facility staff.
  • Stay current with UAS-NY assessment processes and updates.
  • Review and analyze medical documentation and health referral forms.
  • Enter and transmit evaluation data through the electronic evaluation system.
  • Document and address any concerns or discrepancies identified during evaluations.
  • Facilitate communication between the Call Center and eligible consumers for managed care enrollment
