Seeking Experienced Field Registered Nurses
Our client is seeking dedicated and skilled Field Registered Nurses to join their dynamic team in New York City and surrounding areas. This hybrid role combines both in-person and telehealth visits, offering flexibility and rewarding opportunities to provide exceptional care.

Key Responsibilities
- Conduct initial in-home evaluations of patients using the UAS-NY assessment tool to assess health status, strengths, and care needs.
- Develop and maintain positive relationships with consumers, caregivers, and facility staff.
- Stay current with UAS-NY assessment processes and updates.
- Review and analyze medical documentation and health referral forms.
- Enter and transmit evaluation data through the electronic evaluation system.
- Document and address any concerns or discrepancies identified during evaluations.
- Facilitate communication between the Call Center and eligible consumers for managed care enrollment.
Qualifications
- Active and unencumbered New York State RN License
- Minimum of two years in a field-based visiting nurse role.
- Familiarity with the UAS assessment tool (training provided).
- New York driver’s license or NYS non-driver ID.
- Access to high-speed service.
